Sample Code for MVC 4 forms authentication. Any recommendations

Hello. I would like to create a simple login/registration screen for my website using MVC 4, with forms authentication. Authenticating against Sql server.

Does anyone know of a sample project they can share with me, in order to get up and running quickly? I just need solid code that works.

If the sample code only authenticates against username and passwords stored in the web.config file, that is okay. As I can modify the code to authenticate against sql server.

I went to the links, however none of these appears to implement forms authentication? I will download the source and take a look.

I looked through the code in both of those projects. Nothing on forms authentication> Maybe someone else has some recommendations.
