How many Service accounts required for SharePoint 2013 and SQL 2012 installation?

Hi, I want to install SharePoint 2013 so kindly suggest how many service accounts will be required for the installation, below are the details of farm:
There will be two application server, two WFE and one Database server.
All the five servers will be having Windows 2012 OS and will be using SQL Server 2012 for database.
 In addition, four SharePoint servers are virtualized servers only database server is physical server.
So for above scenario kindly suggest the best one and also need suggestion for SQL server installation steps.

Service Account requirements match 2010 requirements.  At a minimum:

-Install acct
-Farm (db access) acct
-Web app pool acct
-Service app pool acct
-Crawl acct
-Profile sync acct
-Super User acct
-Super Reader acct

You'll also want an account to run the SQL Service.
