Approval Workflow for Document Repository

I went into document library settings -> workflow settings -> approval

Created a parallel workflow with 3 approvers.

The fields of the doc lib are:
Name, Category (choice), Country (choice), Owner (Person)

----

Now after uploaded a document, it shows me a blank screen and didn't ask me to enter the remaining fields. This is the first problem.

I received the approval email which is good. But the document gets uploaded straight to the library without even anyone approving it yet. This is the second problem.

In the advanced settings of the list you need to check that Content Approval is required, plus versioning is enabled, rather than just putting the Approval Workflow in place.

Please go into Library Setting -> Advanced Settings and let me know what you find. Same with Library Setting -> Versioning Settings
