No deference, Fitness server is used to host a shared folder called FSW.
what do you mean by this ? when you setup the sharewitness for a dag yes the dag will create the witness folder. i am not sure what you mean though

Yes Suleiman is right the share witness server is used to host the file share witness which is a directory containing a file day member use to prevent split brain

You will need to clarify your question.
The FSW is created by Exchange, however if the server is NOT an Exchange 2010 server then you will have grant permissions on the FSW host to allow the folder to be created by Exchange.
