Making a server map

So my boss tasked me with the daunting job of creating a map or layout of our entire setup here in the office, as far as servers, switches, which server goes to what switch, routers, etc. I've never really done anything like that before or even know where to begin really.

Can anyone offer any guidelines, or even a place to start? I'm not even sure of a way to see all the servers in the office, short of looking at them in the server room, where half of them aren't even labeled. Any starting places or advice on this one? Thanks.

Also, he wants it in somewhat of a graphical format. Any ideas on a program I can use to do this and make it look clean?
