Word Form Creation Queries

Dear Team,

I would like to create questionnaire sort of form for a project . In this I wanted to prepare in word 2010 , with checkboxes, drop down menus etc . I know the basics accessing from the developer and amending it . I have some clarifications related

1. How can I add links to certain drop boxes etc  Eg :if I have three choices , lets say A, B , others , if I select others from the drop boxes, I would like to have the nearby text box activated  .

2. Can I convert the word file to pdf and access / fill the forms there by activating checkboxes , adding text in text boxes etc

Using Content Controls, adding lists is easy for Combo boxes and List boxes.,
On the Developer tab, add your control.  Then click Design Mode and properties (both on the ribbon).  You will get a dialog box where you can add your lists.

If you are going to make this a PDF form, I would suggest just leaving blanks for your controls and then adding them using your PDF tool.
