I am looking for input on monitoring services or reporting tools that reliably measure website page load speed on a global basis.

Any input or lessons learned on which site performance services are most reliable and most cost effective would be much appreciated.

Google's Web master tools include a Site Speed facility, which is based on data fed back from the browsers of actual surfers of your site, and is free. So a good starting point, but will require a quick hack or two to include the necessary Javascript.

If you don't want to embed any tracking code in your site and have money to burn then the likes of nccgroup offer an external monitoring service and provide a  dashboard to view the results.
