Exchange server version: 2010 Ent.
Exchange service pack level: Service Pack 1
Exchange rollup update level: Rollup 5 (KB2582113)

Mobile device model and OS version:
1. Apple iPhone 4s/5 - iOS 6.1.2
2. iPad 2 - iOS 6.1.4
3. Microsoft Windows Surface Pro

Users started receiving the following message in Outlook regarding appointments.

Message:
Subject: Synchronization with your iPad failed for 1 items.
 
Synchronization with your iPad failed for 1 items.
The following items couldn't be sent to your mobile phone. They haven't been deleted. You should be able to access them using either Outlook or Outlook Web App.
 
       Item Folder:      Calendar
       Item Type:      IPM.Appointment
       Item Created:      5/24/2013 12:56:22 PM
       Item Subject:      Meeting with Bill

Update Exchange to SP3 and update iOS to the latest.
Lots of issues with ActiveSync and Apple products which can only be looked at once the products are on the latest versions.
