Outlook on Mac locks AD account

We are having a recurring issue with one of our users. I have tried searching for a solution here but nothing seems to really fit the issue:

The user has a MacBook Pro with Outlook 2011, which is set up with a mail account from our company domain. This is not the user's main work computer, just another way of them accessing Outlook and Sharepoint.

When the user tries to log into Outlook, their account gets locked out on our domain. Our domain controller is Windows Server 2008 R2 and we are running Exchange 2010.

Some side information:

The laptop connects through our corporate wireless (WPA2-Enterprise) but is not a part of the domain.
There is nothing in the Keychain
Mac login username is different from domain login (thought maybe the local login might have caused the issue.)
Accounts lock after five bad attempts
User has not changed their password in two months
Already tried rebuilding user's Outlook profile

Any thoughts would be appreciated, as the current solution is for the user to not use Outlook on their Mac?

Use Outlook Anywhere or VPN solution.

The user accessed via VPN last night with no apparent issue.

No one helped out, so assuming rebuilding the profile manually will resolve the issue.
