Malware - cannot remove Mixi DJ footer from Firefox

Dear Experts,
I have an obnoxious advertising pop up toolbar from Mixi DJ at the bottom of my firefox.

I have removed all the extensions and addins related to it but the damn thing is still there.  I have reset firefox and its still there.

I have downloaded the IOBit malware fighter and its still there.

Any advice on how to get rid of it?
