SQUARE DATA CODE BLOCKS IN ADS????

Most are familiar with the "square code blocks" that one can scan with a smartphone and instantly be directed to whatever website the advertiser wants you to go to. Is there a program that I can use to direct someone viewing an ad in the newspaper to MY website page? If so, what is the utility called? And what is the correct name for what I am referring to as a "square code block".
