How to add call-outs on mouse hover

Hi,

How can I add a 'call out' box similar to the attached picture?

The box appears when I hover over a specific (+) signs and comes with different fonts/etc.

Do I also need special version of Acrobat such as professional?

I currently have Acrobat X.
