Hello,
It's hard to say if this is a technical question or to have a file edited.
In the PDF file you can see 5 imputable rows under the Sequence No, Quantity, description and so on.
I'd like to make more imputable rows, such as 12 total rows
This file was made by someone a long time ago, but I use this file a lot.
I've tried a trail version of fox it pdf editor but haven't seen a way to add more imputable rows.
I'd assume you can edit this PDF file w/ an editor instead of creating a whole new PDF file, right?
I don't think it'd be that advanced to add more rows to the imputable section, but I haven't gotten close so far.

If you are looking for a free solution, try downloading the Nuance free PDF reader and using the built in converter to MS Word. Click on the convert button on the toolbar and your document is emailed back to you in the new format. It may not be perfect, but it should be a good start.

You can try converting it Word and then back again, but the input fields won't be there if you do that. And the conversion is usually terrbile. Omnipage might have something cheaper than Acrobat, but still won't be as good.
