Understanding of Clustered Index

Starting with sample table below, does the index data which follows make sense?

ID              FName               LastName
-----          ------------           ----------------
1               John                     Doe
2               Frank                   Smith
3               Mike                    Smith
4               Adam                  Good
6               Jane                     Brave

index data
ID    FName             ID   LastName
-----------------           ----------------
4     Adam                 6    Brave              
2     Frank                  1   Doe
6     Jane                    4   Good
1     John                   2   Smith
3     Mike                  3   Smith

Question: Is this how a clustered Indexed done?

Here I have all of the columns in the table sorted along with their associated ID (PK).

What is 16 column limit I read somewhere? Meaning each index can include max 16 columns max. If there is such a limit, then what happens if a table has 40 columns? Perhaps this 16 max columns is misunderstanding on my part.

Please comment. I have read many links, unless it has super easy explanations, I would prefer to hear your input.
