IT MANAGEMENT tasks tracking / logins details

Hi,

What type of solutions would be available for an IT team to track jobs that have been done or needs to be done & by who.

Store Policies & procedures
Store password

Open source of course if possible

TeamworkPM is pretty cool, we use it. It's a service, web based and free if you use it only for one project (only counts active projects, the number of users does not count).
Nice integration of google docs, drive, dropbox. You can create Notebooks do document stuff in a Wiki-like markup or use their WYSIWYG editor. We also use it for time tracking. Its not 'made' for IT, but gets the job done.

For passwords you have to stick to the envelops in your save though.
What you are looking for is "Helpdesk ticket software"

Google it, there are tons of different ones out there.  Most are web based.  A lot of them have online demos.

I am not after a Project Management Software or an IT ticket System.

More something like that:

If a technician carries a job on SERV01, I just wanna know what Jobs & by Who have been done to that SERVER.
Attach to that server, I would like to see an option available to see Policies & procedures
