Mvc 4 C#

Hello Team
I am working with MVC 4 C#. I want to create a link in .cshtml that would create a populated popup boxup box like shown in the diagram. I would appreciate if anyone can show me how this is done. Use dummy data If necessary

I tired it but could not get it to work. Does anyone have any examples of how I can do a popup from the @html.actionlink in the view?
