WordPress / Gravity Forms / MailChimp

Hi,

I have a WordPress site with the Gravity Forms Developer Licence. I also have a paid Mailchimp account.

1.      I would like to setup an admin form to enter a person’s name and email address
2.      This would send that person an email and a link to a Gravity Forms survey on my website. This survey link would have a unique token ID. To view the survey the unique token needs to be validated by the survey/website.

What’s the best way of doing this? Is it even possible?

I've used http://www.limesurvey.org/ precisely for that. There is a WordPress plug-in named "SurveyPress" although is a bit dated: http://wordpress.org/plugins/surveypress/

From the Gravity Forms side there's a MailChimp add-on that automatically populates some data in the form: http://www.gravityforms.com/add-ons/mailchimp/
