jquerry form function

Looking for a fix for this code, we had some developers create some code on our new form and want the third question, "Your Future Internet Usage" to drop down after the second question from part two (How do you currently receive Internet at your home?) is check off via the radio buttons.

I do not know if they have the code separated by class or if I can just change one variable in the script for that section to allow for this to happen. We need a fix soon to replace our old website page....

sorry, yes you need to fill out all form fields in part one and use both drop downs to allow the next question to appear.

you need to fill out name address and all other fields... including

name
housenumber
email
streetname
town
street type dropdown
street direction dropdown

second question will appear after the second dropdown is selected and all form fields are filled out
I don't find that form intuitive and I'm web savvy, that there are textboxes there to be filled in is almost impossible to see.
Looking to brush up on my code can you recommend a good code book for this type of script?
I'd say the web is your best reference, any book you buy will almost always be out of date
thanks Gary I will have the designers move the "Town" form field and also try and use a more visible font colour.
Maybe make the textboxes have a darker border and lighter watermark
