PHP or JQuery Calendar script

Is there a PHP or JQuery Calendar script that can be used with a Form?
Meaning if I have a form:
<form>
NAME: <input type='text" name="name" id="name"/>
TITLE: <input type="text" name="title" id="title"/> 
DATE START:
DATE END:
</form>

At DATE START and DATE END, have a PHP or JQuery Calendar popup that I can pick any date in the future and that will be included in the MySQL INSERT Query.

Does that make sense?

Here is a basic working sample based on JQueryUI
