That's close, but still doesn't look quite like the image I posted.

If you want it that precise, then take the code from the posted link and play with it.  It is not likely you are going to find the exact presentation you want when you decide on a particular look.

I'm not sure how to incorp the image with the page itself, that's why I'm asking here.

As an image it is not functional as a select.  If you want a select input box to look like that then start with the code posted by HagayMandel and play with the CSS to get it the way you want it to look. There is no magic; just work.  If you are not willing to invest the time in learning how to do it, then you just have to work within your limits.
