If javascript is disabled, pdf crashes Safari when interactive form is populated

I have an interactive pdf at https://secure.mauitradewinds.com/TransMan/resform.pdf    I've tested it in the major browsers, with and without javascript enabled, and all seems well, with one exception.  In Safari, with javascript disabled, the browser crashes if I attempt to populate a form field.  This happens on two Windows 7 desktops running Safari 5.1.7.

I know that the percentage of visitors who will use this form, and have javascript disabled on a Safari browser is small.  Still, if there's a simple fix, I'd like to apply it.   The same crash happens in Safari (with javascript disabled) with any interactive pdf on any website, not just my file.

I am not aware of any workaround or fix. I would check for the browser and if JavaScript is enabled and either offer a different web page with information about how to fill out the form (e.g. download and open in Adobe Reader instead of filling it out inside the browser, thus avoiding the crash).
