Give us an example of what you're trying to pull off here, as there could be multiple solutions.

The literal answer would be..
CREATE PROC YourProc AS

-- code goes here

ALTER TABLE SomeTable
ADD id int IDENTITY(1,1)

If you meant adding sequence number to output being presented, you can use
ROW_NUMBER() OVER(PARTITION BY YOUR_COLUMN ORDER BY YOUR_COLUMN)
