Reports in Access Database

I am trying to make a report in access database.

I am looking for a output as below
      Jan-13      Feb-13      Mar-13      Apr-13      May-13
New      20      3      20      1      15
Increase      45      10      15      30      30
Refinance      9      4      5      8      5
Total      74      17      40      39      50

But what I am getting is totally different
New      20
Increase      45
Refinance      9
Total      74
New      3
Increase      10
Refinance      4
Total      17
New      20
Increase      15
Refinance      5
Total      40

Can you please help me out what I am doing wrong?

Have you tried using a crosstab query?

If not, use the query Wizard to create as crosstab query using your original query.

Yep I am trying that using a wizard still not getting enough success as I have ask for the
parameters for limiting the month and it should give me the application type and then count that application type . I am really stuck on this right now

TRANSFORM Count(Applications.[Customer]) AS CountOfCustomer
SELECT Applications.[Application Type ID], Count(Applications.[Customer]) AS [Total Of Customer]
FROM Applications
WHERE (((Applications.[Application Type ID]) In (1,2,6,11)) AND ((Applications.[Decision Date]) Between [sd] And [ed]))

Crosstabs handle parameter different.
