Look at the logs of postfix and the user's configuration to see what is going on.

The sending server will resend the message so long as it does not receive the 2xx message when it indicates the end of message.

There are ways to deal with duplicates.

Is every message to this recipient has duplicates, or is it when the size of the message is larger than x?

Check with /etc/alternatives aswell.

Also check the mailq and mail server config... it should not be open relay.
