Create a Submit button in Word 2013 Form to email as an attachment

I am new to creating Word 2013 forms.  I would like to create a form button that when pressed, will email the current form to the specific people.

I would also like the Subject of the email to be pulled from one of the form fields.  i.e. Project Title.

What are the steps or code to accomplish this?
