Programs icons show in taskbar but will not open

Has anyone seen this problem: multiple programs open and show in taskbar but unable to open them.  When I skim over the icons in the taskbar I do not get a preview.   The icon flashes quickly.  I am unable to open programs that are open.  I have to right click on the icon in the taskbar and select close to close all the Windows and reopens via the file.  In the case of IE, I right click, select close, and when the window opens hit cancel and then we can use IE.   ALT + Tab doesn't work to move between program either.
Note: This happened a month ago and the laptop was replaced. Now it is happening on the replacement.
Any help would be greatly appreciated.

Check the display settings, make sure the resolution is all the way to max setting.  Also see that it's not detecting more than one screen.

I agree, this could be a matter of the windows displaying, but being outside the viewable area of the monitor.  Does any action precede this problem?  Does the problem show up after you run a program or a game?  Maybe something being run from a command prompt?  Do you change the resolution of your monitor for any reason?

The laptop is docked and does use multiple displays.  Resolutions are set to the max.  I will test to see if it works when undocked.
@paulmacd It just started when the user docked her laptop (but she has been doing this for a month).  No other change reported.  The icons don't even let you preview them when you hover over.

The preview thing is usually theme related, so try changing themes also?

In addition to the theme suggestion, does the dock have its own monitor?  Is it of a different resolution than the laptop's display?

The speaker volume on the quick launch bar was stuck on increasing the volume.  Had her press on plus and minus.  The volume bar then went away.  She was then able to see what progams were open.
