I wear a lot of hats - Developer, Database Administrator, Help Desk, etc., so I know a lot of things but not a lot about one thing. Experts Exchange gives me answers from people who do know a lot about one thing, in a easy to use platform." -Todd S.

Ok, I got that part and I can see the data fields that I can select from when I created the data connection and then the form.  What do they mean by NTLM authentication credentials?  i did not see anywhere in this process that I had to add any authentication credentials.  I am getting an error when I publish this but was wondering if that is because of the credentials issue?

I am assuming you want to use static NTLM credentials?

You have to go to your UDC library.

Go to the dropdown and use the Send To and download a copy.

I believe that by default there is no authentication in a UDC and it assume to pass windows credentials.

You need to add this to your text in the UDCx xml.
 <udc:Authentication>
        <udc:UseExplicit CredentialType="NTLM">
               <udc:UserId>svcPortal</udc:UserId>
               <udc:Password>p0rt@ludc</udc:Password>
        </udc:UseExplicit>
</udc:Authentication>

This block is after the /udc:UpdateCommand and before /udc:ConnectionInfo elements.

Just add your username and password that you wish to use.
