Custom Form in Outlook

Hello Experts,

I have created a custom form in Outlook by going into one of my contacts, going under Developer and using Design This Form.

This works well for this contact only, but my question is how can I apply this form to all of my contacts?

Hi,
following the steps in this MS article should do the trick:

Please also do not forget to set the new form as the default when posting new items to the contact folder (see Figure H on this site):
