I have a user that keeps getting this when they use outlook. What is it? How can it be removed?  We use exchange 2013.

Microsoft Office 2010 and later versions as well as Windows Shell (Web Folders / miniredirector) requires secure SSL connection when used with Basic authentication. Microsoft Office 2010 will fail to open a document via insecure connection with Basic authentication. For a workaround please see this article: You cannot open Office file types directly from a server that only supports Basic Authentication over a non SSL connection with Office 2010 applications.

user / developer can take a look at the respective servlet configuration
in the WEB-INF/web.xml and uncomment the init-param
named "missing-auth-mapping".

then you may choose between option 2) and 3) of
the description.
2) empty param-value -> null login -> read-only
3) userassword value -> login as that user
