I'm tring to print and no matter how I seem to tweak the settings, I can't get th headers and footers to properly appear on the page (see graphic).

What is so difficult about fixing what is seemingly a dumb problem.

It looks like part of both header and footer are outside the page margins, which may mean that your printer won't/can't print them. (Some printers can print outside the page margins, some can't.)

Try fiddling with the margin settings to move the header and footer farther from the edge of the page.
