Ok i used the "contact form 7" the first time to make a form for wordpress and with the help of an expert here I got it to work.  Now the individual that im building this site for has some more requirements for the form..

Now to attempt this feat i installed "Fast Secure Contact Form" and configured it.

Now after configuration and adding the short code to the contact us page im getting the exact same issue as before where the input fields are now showing up.

heres the link to the issue: http://logoshealthcare.com/contact-us

The problem is not with the plugin, it's with your CSS file for the theme.  You have the following entry in style.css

* {
    border: medium none;
    outline: medium none;
    text-decoration: none;
}

Open in new window

That style unsets borders, outlines, and underlines for every element on the page.  So the fields are there (just click on the area to get the cursor to focus in), you just don't have any visual cue and you need to either remove that entry (not advised) or add entries that turn borders back on for form elements.

well I already have:

input.wpcf7-form-control { border: 1px solid #c4c4c4 }

and

.wpcf7-form-control { border: 1px solid #c4c4c4; }

in my .css file and nothing.

any other suggestions on what i can use to force the fields to show?

Right...because removing the * disables the style rule.
