AD/Exchange - Double AD object with same email address

Dear Experts,

Can you please have a short look on that attached picture:

Active Directory double objects
it is strange looking case when Active Directory shows two objects for the same one created person, with a long CNF:.. kind of text in the Name.

Assumingly the system generated that automatically, have you seen maybe such one? What can be the root cause of that?
Checking the email address in Exchange 2010 that is also strangly showing two lines for one email address, but that so should be definitely unique in Exchange

Thanks for that article, it is absolutely possible in our environment:

"How Duplicate Objects Can Be Created
It is possible to create duplicate objects (with the same RDN in the OU or the same sAMAccountName). This happens if the objects are created by different people at almost the same time while they are connected to different domain controllers. If the new objects are saved on different domain controllers before replication makes the conflict apparent, no error is raised. This will most likely happen if the objects are created on domain controllers in different sites, where replication may take a few minutes. However, it can even happen when the domain controllers are in the same site and well connected"
