Capacity management guide for Cloud VMWare

Anyone has any capacity management guide.  Ideally it touches
topics like

a) at what utilization of a datastore, we need to consider increasing
    the datastore's capacity
b) when to increase vCPU, disk & RAM for a VM
c) how do we chart / track the various resource utilizations
d) when's a cluster & ESXi host's total CPU & RAM needs to be increased
e) how would we know increasing vCPU & RAM will improve a VM's
    performance

Does vCloud Director do capacity mgmt?

a) when the datastore get's less than 20% of the total storage, but is dependant upon number of VMs on the datastore, and swap file location, and snapshots.

b) Check Performance and Statics of the VM, check performance charts or use VMware vCenter Operations Manager.

HOW TO:  Performance Monitor vSphere 4.x or 5.0
