I have an old iPhone 4 that I used while on Verizon Wireless. Can I activate the phone on AT&T and utilize it? If so, how? Jailbreak it?

If it is the same kind of phone (CDMA vs. GSM) and uses the same SIM card, then you should be able to get a new SIM from AT&T and set up a new accounts. Make sure there is no contract on the old phone. There should be no need to jailbreak the phone.

Yes. My iPhone has a SIM card and it can be replaced. You need to check this with Verizon and AT&T.
