Background:
IBM Storage DS3512 and HyperV 2012 , two hosts in Cluster Failover
about 4 VM`s (Server 2012) on each host

host Operating system set to Windows clustered

Have done a Firmware upgrade no result.

get the Logical drive not on the preferred path , no changes or work in the server room , cables had been checked by onsite person , all looks ok , can change it manually to correct preferred path after a few minutes just fails , currently only two Volumes of about 8 total ones does that.

Not sure why :
Have engaged with IBM support too on this they are looking at the Logs now:

Any help on this please:

Have solved this issue
Process followed  IBM SAN and HyperV 2012

1. Logged incident with IBM

1.1.)look at the host configuration of the iSCSI initiators on the host and/or the configuration of the iSCSI switch. (the vlans may not be configured correctly)  This was correct

1.2)You should change you host type accordingly if you are not using Symantec Dynamic Multi-Pathing drivers for failover.
You currently have the host types defined as Type: Windows Server 2003/Server 2008 Clustered (supports DMP)
Windows normally uses MPIO drivers for failover (ie non DMP) (for details see you windows documentation and or speak to you related support person)
this was correct

2. Checked everythin related to  HYPERV on the Host
2.1. discovered the Storage adapter on one of the hosts dissapeared from the device manager and networking managemenent

Was lucky > restarted host and Card showed up in windows again

Subsequently , Firmware and driver updates needs to be managed on the host....

Issue been cleared for a month now
