Add date/time not working how to fix

Seems like the most simple thing but its not working in this database.  I need to be able for access to add the date and the time when I add the date to a field.  See the attached database.

It is on the notes table.  But it is added from the Contact Form under notes.  I need to add the date and have it add the time the date was added in that same field.   I know this is simple but for some reason it is not working.

if you are looking for default current date and time, you can set the default to Now()
or if the user can edit the date field, you should add a new field "CreateDate" and set the default to Now()
