PDF Form saved and emailed to databound recipients

Hello,

I have a client who would like to be able to upload a PDF Form which would display on her website. After the form was filled out, it would be saved to the server, and a copy of the PDF would be emailed to a conditional client list pulled from a database.

I'm not sure how to approach this. Can I get some guidance here?

Page_Load()
{
     Get And Display The Needed PDF Form
}

ButtonSubmit()
{
     Save the PDF form as some unique name
     Attach and send the PDF to an email to a list of recipients called from our database
}

I ended up using dynamicpdf merger from cete software as recommended. It is awesome.
