Form fields are disapearing from by adobe pdf

I have an adobe PDF that i added form fields to.  Why i open it up I can tab through and enter information.  But, when i open if from my software application the form fields are not there on the form.

Any idea why?

What is your software application? Does it support interactive pdf's?

I was using dynamic.pdf.  I figured out what i needed to do in the code
