How do I configure Outlook on iPhone 5?

Please note these devices were working fine and now there is no mail in the Inbox but mail does appear in Sent box. I have reconfigured the phones several times. I have recreated the accounts on the Exchange server several times.

Are you adding an Exchange account to the iPhone or some other type of account?

Exchange is what you want to be adding if you have an Exchange server.

Also - just for reference, there is no Outlook on an iPhone - just the Mail App.

Perhaps my verbiage is incorrect. My apologies. On the iPhone, you can configure an Exchange account for email. I have an Exchange 2008 R2 Windows server. As I stated previously, the account was working until a few days ago.

Okay - what version of Exchange do you have?

Windows 2008 R2 is the Windows version.

Have you tested the account on https://testexchangeconnectivity.com?  If not, please do and let me know the results.

This account is working from both desktop and laptop. Just having the issues on iPhones and Androids.

That's good - but mobile phones use a very different form of communication, so whilst Outlook may work happily, and mobiles should too, that fact that Outlook works, doesn't mean that the mobiles will.

Can you please visit the link above and run the Activesync test and let me know the results.

Okay - so it looks like there is no Autodiscover DNS record setup to point the world to your server, which your mobile phones will need (and so will Outlook for out of Office).

Did you install an SSL certificate onto your Exchange server when it was installed?
