How can i build a simple form in joomla with several Radio Buttons

Hi there,
I need to create a simple form with 5 radio buttons. Once it gets presented to the user and a button gets click based on their previous selection on the radio buttons the user get redirected to a different link.

something like this:

o1
o2
o3
o4
o5
 
Submit button => once it gets click if the user select 1 then go to www.google.com if user selects 2 then go to www.yahoo.com ... until the last option

How can i do this on an article?

and it seems to be doing exactly what i need
