Any idea how to create a survey on Portal and add it into the left side of the SharePoint content ?

You can add a List Web Part displaying your survey anywhere you can add a web part normally.

If you have navigation on the left you're not going to be able to drag a web part there. (But if not it's fine).

If you dont like the way out of the box surveys look, it's possible to embed third party surveys like Survey Monkey onto your pages;
