Multiple ICA session for the same user and the Same Application

I keep seeing Multiple ICA sessions being created for some of our users at random, I have tried everything I can think of to resolve the issue but so far both myself and Citrix have not been able to find a viable fix for it. It started shortly after upgrading to Rollup Pack 2 for XenApp 6.5.

What happens is that you will see the Same ICA-TCP Name, Same User Name, Same Application Name, Same Session State, and the Same Client Name but both are on the exact same session, one will show the idle time as being blank while the other as being active, yet per the user they are not able to properly use the application.

I have already reviewed that thread and I have already validated that the suggestions on that particular thread is already in place. The thread was an issue in the Citrix 4.5 environment but we are running Citrix XenApp 6.5. Thanks
