Accord5 Trellis software with blank index.php

I have currently inherited a php/MySQL beast of an application that is not longer supported or being developed against. I had the program working great, and did an upgrade to a new server last night, the only difference is the IP address, which I believe I have taken care of currently. I can log in and administer the "software", but the main user sign in page index.php is black. I have it isolated down into an include file that it calls, but I am at a loss from that point on, I just can't seem to narrow down the issue.

Has anyone used this product, or can point me in the right direction to finding a solutions.

If you move to a new server, it is possible the new server is running a newer version of PHP that no longer supports some of the code in the include file. For example if the include is using the old mysql API for DB access and the new server is running PHP5.5 you will have to convert to either PDO or the mysqli API.
