First at all let me say, that ou should keep several permissions in mind, the fist is sharepoint itself, where he user may need permissions, and the second is the file system, which is liked into sharepoint´, and third possibly the identit, which interacts in this procedure.

The user needs the permission in sharepoint.
The user (or possibly one of the service accounts) need the permission in the file system.

Ypu may get rid of the issue by the following logs.
1. The ULS logs of sharepoint (you may download one of the ULS log viewr from codeplex).
2.) The trace log in IIS as stated in the article
3.) the IIS logs themselves.

This way you may get rid of the user who tries to change an item (what should usually be the user who is doing it). Usually means, in a farm constellation it can even one of the used service accounts.

So follow up the event logs of IIS to get rid of the users and the ULS logs, why an action fails. But usually would aaume the NTFS permissions in the file system
