Performance review - how to make it electronic?

Hi,

I've been assigned to transfer our Perfomance review sheet and make it electronic - I thought about some sort of a survey or Google docs but not really sure how to do it as user needs to fill in the details or answer questions, then it goes to the supervisor and GM and then throughout the time, there can be adjustments for that period.

I have access to SharePoint as well so possibly could do it there but any suggestion to software, ideas how to do it are appreciated.

Both google docs and sharepoint have a way to make a form and store the data.  I do think google docs is much easier to use, but if you have sharepoint and it is actively used, that may be the better choice to keep  your data in one spot http://office.microsoft.com/en-us/sharepoint-designer-help/create-data-forms-using-sharepoint-designer-HA101631630.aspx

Using google Drive/Docs you will just click the big red create button, choose form and the wizard will guide you

I realised that Google Docs probably won't be a possibility as it's not stored on our server and just wouldn't go well with the management.

With the form on SharePoint - will they be possible to review it and edit the entered data later?

I have only used google drive for this type of thing.  I have used sharepoint for file sharing.

You can set up your own work flow.   Sharepoint allows you to do more, but that means it will take some more planning into sett up.  However, there are some premade template you might be able to use to make your job easier.
