Quickbooks 2014 Plus Annual Subscription - Ongoing license issues

We recently upgraded our 3 User Quickbooks to the 2014 Plus edition on Annual Subscription for support of Windows 8. It ran without issue for about 6 months. Then we began getting messages about our subscription being expired (it wasn't) or that our credit card information was no good (it wasn't either) - Quickbooks would run but say you only had X days left to fix the problem.

Since the subscription comes with unlimited tech support I made the call.  The information I got back from them solved the problem that day, but it keeps coming back every week or two. It will start with one user, then eventually affect everyone.

The temporary fix is simple;  Delete the file C:\ProgramData\Intuit\Entitlement Client\v8\EntitlementDataStore.ecml which will force a fresh registration. Intuit provided me with a 6 digit code that can be used over and over again to register our copy.

So, the issue is not how to resolve the error, but rather why does it keep recurring?

Full disclosure:  We have a 3 user license, but have the software installed on 6 workstations. Only 3 users access at any one time. When we purchased QB 2009  Intuit told us this was fine as they uses a concurrent licensing scheme. I have a sneaky feeling that they have modified their scheme recently and that may be causing the problem.

I will check machine dates on Monday.

99% of the time they close QB each night. We have an automatic backup of the QB server. Once every few month's I'll get an error that files are left open, but it doesn't seem to correlate with the license issue which is more regular.
I had a chance to check the time settings on all of the machines being used for Quickbooks. Everyone was within 12 minutes of correct date/time/timezone. I updated them all to be the same within 1 minute.
