I am looking for template to developing a policy for cloud IaAs that also includes approval process/ownership.

It sounds like you're looking for a workflow solution.  Depending on which vendor(s) solutions you choose, there is likely a workflow tool available.

For example, Citrix has App Studio, which enables you to create workflows.  Although Citrix recommends using it with Cloud Portal Services Manager, it is not required.

And it is owned by your cloud provider, not you.

For 500 point I would think we can do better. I am looking for a written policy I can use to control the commisioning of servers in the cloud Iaas which we host onsite using the HP Matrix cloud solution.

Your last post is whole policy...
Just add some sensible limits say max 50pct off site and you have written policy to review as need arises or yearly.
