I have a Windows Server 2008 R2 KMS host (for Windows 7 and Office 2013).  I have about 60 Windows 7 desktops that utilize the KMS host for OS and Office 2013 activation.  OS activation is fine, but I am having an issue with the Office 2013 activation.  All these desktops are clones from a parent image, so I thought I might be having an issue with duplicate CMIDs, however, I just ran slmgr.vbs /dli on 6 different desktops (including the parent image) and ALL are showing unique IDs.  If I run cscript slmgr.vbs /dlv 2E28138A-847F-42BC-9752-61B03FFF33CD on the KMS host, I see the following:

Microsoft (R) Windows Script Host Version 5.8

What I am concentrating on here is the current count of only 3.  I have run cscript ospp.vbs /act on all 6 previously mentioned Win 7 desktops, and I cannot get this count to increase.  Here is the output of this command:

Most of the KMS activation issues are because of missing or incorrect DNS SRV record. http://blogs.technet.com/b/odsupport/archive/2011/11/14/how-to-discover-kms-hosts-via-a-dns-query-and-remove-them-if-need-be.aspx However, since the clients contact the server, the DNS seems correct.

The KMS activation diagnotic tools could help http://technet.microsoft.com/en-us/library/ee624355(v=office.15).aspx

You can also try to reset the Office 2013 KMS GVLK http://technet.microsoft.com/en-us/library/dn385360(v=office.15).aspx

Ok I used the KMS host diagnostic tool, and after looking at the KMS event logs on the host, I saw that in fact I did have some clients with the same CMID.  So I ran a ospprearm.exe on the parent image, created a new snapshot, and recomposed the associated View desktops.  It looks like it's going to work now.  I will verify in the morning.
