Mailbox access major issue

See attached file for message MapiExceptionSearchEvaluationInProgress and unable to create folder
When we create a user we sometimes get this issue and the fix is to delete the user and mailbox and start all over again.

If we create a mailbox and the initial login to that mailbox is via outlook, normal outcome. If the initial login is OWA we get this error  MapiExceptionSearchEvaluationInProgress  Unable to create folder. (hr=0x80004005, ec=1177)

This seems to be the issue (KB94116), note that the OWA information from both experts was also extremely useful on background.
