Can someone please provide a Security Awareness Training Policy Acknowledgement template?

I need to include a written Policy Acknowledgement. This will be available for our users to read and acknowledge
our Security Policy/ Acceptable Use Policies regarding content,security, email etc. as part of the training course. I need a template (not to long of a policy) letter that I can apply for my organization.

You can get a little blurb from your legal department (or your HR department if you have no legal) about what would happen if someone was caught violating 'the above policy' and append that to the policy.  Print it, have them read it, sign it, file it.  Easy peasy.
